ABOUT THE ROLE


Our dedicated Support Coordinators manage a caseload of NDIS participants, providing quality support coordination services to assist participants in understand their plans, selecting and engaging service providers and achieving their personal goals.

You will be joining a small Support Coordination team with plans for growth in a dynamic organisation.

Reporting to the Team Leader - Support Coordination, this role is due to commence early July 2023, remunerated from $77,578 per annum (SCHADS Level 4) plus leave loading, superannuation & salary packaging.


  • Responsibilities: _
  • Managing a caseload of NDIS participants with psychosocial disabilities;
  • Building the capacity of individuals to navigate, understand and coordinate their NDIS plans;
  • Supporting individuals to reach their NDIS goals by connecting them with appropriate community, mainstream and government services;
  • Developing Service Agreements & implementing funded supports from a participants' NDIS plan;
  • Facilitating choice and control, enabling individuals to lead lives that are meaningful to them.

ABOUT YOU

  • Essential Criteria: _
  • Experience in Support Coordination or similar case management role
  • Knowledge of NDIS processes and guidelines
  • Experience with & passion for supporting people with psychosocial disabilities to achieve their goals
  • Excellent interpersonal communication skills, both written & verbal, and experience building trust and rapport
  • Excellent attention to detail, record keeping, time management and process orientation
  • Demonstrated consumerfocused and resultsfocused orientation/approach
  • Demonstrated high level of discretion, as well as a robust understanding of, and commitment to, professional boundaries.
  • Ability to work independently and unsupervised
  • Intermediate skill with MS Office suite including Word, Excel, Outlook and databases
  • Current NDIS worker check (or willingness to obtain)
  • Current first aid certificate
  • Current driver's license
  • Unrestricted right to work in Australia
  • Desirable Criteria: _
  • Previous experience working in a notforprofit organisation
  • Relevant tertiary degree or equivalent (e.g., mental health, social work, community services).
  • Understanding and commitment to a trauma informed, recoveryoriented, personcentred approach.
  • Mental Health First Aid
