About this Role


As Regional Assessment Officer, you'll support our Regional Assessment Service Team in completing home based assessments to determine eligibility for entry level government funded aged care services.

You will assess an older person needs, goals and preferences and refer them to services that will help them achieve the best possible level of function and independence.


About you
For this position we are seeking with excellent communication skills, who can work collaboratively in a team environment.

You will have a strong commitment to customer focus, with the ability to quickly build rapport with clients as well as building relationships with external agencies to enhance client outcomes.


You will also have:

  • Relevant Tertiary qualifications in areas such as nursing, allied health or social work and experience in community aged and/or disability preferred
  • Previous relevant experience working in a similar role within Aged & Disability services.
  • Sound knowledge of the Commonwealth Home Support Program guidelines and priorities
  • An understanding of the My Aged Care and Ereferral systems for the assessment and referral of clients to other agencies.
  • Competent computer use skills specifically Microsoft Office, internet based programs and capacity to learn service specific systems/software.
  • A current Victorian Driver's license is essential


All successful applicants will need to successfully obtain a Police Check and hold a valid employee Working with Children Check.
