Position Objective:

***The Care Management Officer is responsible for the efficient and effective provision and development of Care Management services, to ensure that the assessed needs of eligible frail aged people, people with dementia, people with disabilities and their carers within our community, are met in accordance with City of Greater Geelong policies and the Home Care Package Program guidelines.


What We Need:


  • A tertiary qualification or demonstrated experience.
  • Proven experience managing issues relating to frail aged people, people with dementia, people with disabilities and the carers of these people.
  • Sound knowledge of the Home Care Package Program (HCPP) and the Commonwealth Home Support Program (CHSP).
  • Knowledge of government and community resources available for frail aged people, people with dementia, people with disabilities and their carers.
  • Comprehensive knowledge of holistic assessment and care management practices.
  • Program planning and development skills.
  • Research and evaluation skills including data collection and analysis.
  • Ability to work within limited funds whilst providing best practice care outcomes for clients.
  • Knowledge of Carelink (desirable).

Main Focus:


  • Develop and maintain effective referral, assessment and review mechanisms and care plans to manage the needs of prospective and existing service recipients.
  • Implement care management practices which promote privacy and confidentiality.
  • Ensure appropriate planning, provision, coordination, integration, development and evaluation of Care Management Services.
  • Respond to new clients and prioritise for a range of Community Care Services within two working days.
  • Provide and maintain uptodate knowledge of developments in Care Management Practices, management of the frail aged, people with dementia, people with disabilities and their carers.
  • Ensure clients and their carers are provided with a copy of all assessment documentation and informed of their rights and responsibilities, type and level of care to be provided, the costs involved and grievance procedures.
  • Ensure consistent, effective and efficient administrative systems are maintained. Ensure regular collection and reporting of statistical and other data.
  • Effectively manage Home Care Package budgets ensuring that services and supports are managed within the limits of the package whilst also meeting clients' needs.

  • Fulltime hours 38 per week.
  • Shortterm temporary position.
  • Hourly rate $57 per hour
  • Work is in Geelong CBD (some work from home).
