What you'll be doing

The Social Worker will provide a high quality, client-centred social work service to clients of the Blacktown Transitional Aged Care Program.

The Level 3 Social Worker is able to operate autonomously with minimum direct clinical supervision. They are involved with planning, implementation and evaluation of TACP quality improvement initiatives and policies and procedures.

Essential Requirements

Licence:
Current NSW Driver's Licence valid within Australia

Qualificaton:
Tertiary Qualifications in Social Work and eligibility for membership of Australian Association of Social Workers (AASW)


Selection Criteria

  • Post graduate experience in the delivery of Social Work services in the area of Aged Care and/or Rehabilitation in a hospital or community setting with demonstrated understanding of the needs of aged care clients. Management of available resources to meet demands of the service and communication with hospital personnel, in order to provide a smooth transition between inpatient and community care
  • Demonstrated ability to undertake comprehensive social work assessments, to formulate and implement appropriate care plans for aged care clients
  • Experience in supervision of professional staff and/or senior students
  • Welldeveloped time management skills with excellent written and verbal communication, including report writing skills
  • Demonstrated ability to work with mínimal supervision and as part of a multidisciplinary team
  • Demonstrated experience in quality assurance and / or clinical practice improvement activities
  • Current NSW Drivers licence with good driving record
