We support people with physical, intellectual and multiple disabilities and developmental delays to achieve their goals Scope is a registered provider under the NDIS and delivers a range of services including supported independent living, short term accommodation (respite), lifestyle options and therapy across 100 locations in Victoria.


About the role
We are seeking an enthusiastic Support Coordinator to join a skilled and motivated team.

At Scope Support Coordination we are committed to providing high quality Support Coordination services, where we collaborate with individuals and families living with disability to strengthen their ability to navigate the NDIS and coordinate their supports as well as participate more fully in their community.


Our exciting and unique opportunity is for a Support Coordinator to join the Support Coordination Victorian team on a permanent full time basis.


  • Manage and implement participants' NDIS plan
  • Achieve the outcomes identified in the participant's plan
  • Connect the participant with formal, mainstream, and funded supports

About You

  • Tertiary qualified, or equivalent in a discipline related to disability supports or 2 years experience working within the disability sector
  • Experience supporting adults and children with complex disabilities utilizing a range of personcentered service models
  • Experience in support planning and / or coordination
  • An understanding and sensitivity to the rights and dignity of people with disabilities in line with current legislation
  • An understanding of the NDIS and individualized funding
  • Ability to work with mínimal supervision
  • A high level of organizational and time management skills e.g. data collection, reporting
  • Excellent time management skills
  • Advanced computer literacy skills
  • Ability to work with a variety of stakeholders
  • Excellent written and verbal communication skills, including report writing

Requirements:

To be considered for this role, you must provide the following:

  • Satisfactory Working With Children's Check (Employee)
  • NDIS Worker Screening Check Clearance
  • Proof of 3 doses of COVID19 vaccination in accordance with Scope policy
  • Valid Australian Working Rights and an International Police Check if you have lived or worked overseas for more than one (1) year within the last ten years
