This Case Manager position pays up to $75,000 plus benefits.- A leading community care provider is currently seeking a Case Manager to with aged care experience to join their dedicated home care team in Northern Beaches.

The ideal Case Manager will possess the following:
- Certificate IV in Aged Care/Community Services or equivalent (minimum)
- Previous experience in Home Care/Community/Aged Care
- Current driver's license
- Intermediate computer skills including MS Office #health
