About the Women's and Children's Health Network
The Women's and Children's Health Network provides state of the art health care for women and children in Adelaide and South Australia.
We are the state's leading provider of specialist care for children with acute and chronic conditions and SA's largest maternity and obstetric service.
Metropolitan Youth Health provides free clinical health services (primarily for physical health issues) to young people aged 12-25, with a focus on under 18 years, in the Adelaide metropolitan-wide community.
We have a grant funded part time opportunity for a qualified Social Worker to join the MYH team - in the Talking Realities Flexible Learning Program (Supporting Young Parents)
In this role you will be responsible for:
- Engagement of young pregnant women and /or parents, who are dis-advantaged and/or vulnerable to access and navigate through specialised primary health care services.
- Delivery of SACE and VET accredited learning packages as part of the Talking Realities Flexible Learning Program
- Development and delivery of quality outcomes for clients through the provision of youth health care services within a case management framework, aligned to the Metropolitan Youth Health Model of Care and the Department for Education Flexible Learning Initiative.
- Delivery of group programs and 1:1 interventions
- Improved service access and outcomes for young people, young pregnant women and young parents through working in partnership with them and their families and in collaboration with other health professionals and government and community agencies.
- Continuous service improvement through research, evaluation and the development of, policies, procedures and practice standards for the organisation.
- Provision of quality supervision and professional development to team members.
