Brightside Recruitment has partnered with an established service provider who offers a range of specialist services including community outreach, recreational and residential services, ensuring their participants receive the support they need to live independent and fulfilled lives.


Role:


Based in Sydney's Eastern suburbs, the Care Manager will manage allocated cases within Home Support Services including the management and supervision of Home Care Support Workers, entering services into the scheduling system, managing invoices, and liaising with other services providing care and assistance to clients of the service.

Role will require;

  • Provide case management services to clients to ensure provision of appropriate services to meet individual client needs;
  • Complete a comprehensive assessment with the client to assess strengths and needs, as well as a risk of harm to the client in the home;
  • Develop case plans with clients and assist clients in the development of problemsolving strategies that foster independence;
  • Provide inhome supportive services; community and neighbourhood resource linkage and referral; advocacy; and other social service intervention to clients within the context of their community; assisting the client in creating firm communitybased linkages by initiating the relationship between the family members ad the provider;
  • Respond and manage crises in a timely manner, being calm, focused and ensuring that the needs of clients are met;
  • Negotiate with service providers creating innovative opportunities/services for clients;

Essential Criteria:


  • Tertiary qualifications in Social Work, Social Science, Allied Health or Nursing;
  • Extensive knowledge and understanding of the needs of aged clients and their carers;
  • Extensive experience in the Aged Care sector or related work;
  • Demonstrated experience in case management in the aged care sector;
  • Knowledge of Consumer Direct Care packages, programs and standards;
  • Ability to assess client support needs clinically and develop care plans that support those needs;

  • This role is immediately available
  • Full time hours, Monday to Friday
  • A successful NSW Working with Children Check (WWCC) and Federal Police check is required prior to starting in this role
  • You must be vaccinated against Covid19
