  • To provide case management support in a community setting, supporting individual to secure housing, experience improved social inclusion and achieve housing stability,
  • This support is achieved through personcentred case management incorporating resilience, traum informed and strengthsbased practices whilst working effectively with other members of the team to provide a comprehensive service that reflects the vision, mission and values of The Salvation Army.
  • This role is within the Homelessness Stream of the Social Mission Department.
  • Establishing relationships with individuals that access the service that encourage selfdetermination and participation
  • Providing case management support to individuals in accordance with TSA case management models and standards of practice
  • Ability to complete thorough assessments, inclusive of needs, risks and vulnerabilities
  • Ongoing assessment of the individuals' presenting issues, vulnerabilities and needs from initial intake to exit
  • Individual case planning in collaboration with the individual including a range of interventions that address the needs or goals of the individual, as well as barriers to achieving those
  • Implementation of case plans with active individual participation
  • Assisting individuals to access support services and establish community supports, and where needed provide advocacy for the person with service providers
  • Regularly reviewing progress and outcomes with the person of individual case plans and where needed taking action to ensure the plan continues to meet the persons needs or goals
  • Maintenance of accurate, up to date records and data in accordance with TSA standards and procedures using TSA information systems
  • Identifying risks to individuals, staff and TSA and addressing where appropriate of bringing to the attention of the Team Leader as soon as possible
  • Service gaps are identified and recommendations for service delivery improvements are made to the Team Leader
  • Participates in local network meetings inclusive of Communities of Practice meetings
  • Displays high levels of emotional intelligence, with an awareness of how personal values and beliefs can impact work
  • Participates in regular supervision and case review meetings
