Your New Role:


Join a fabulous values-based culture as a
Team Leader - CCCP where you can make a positive impact providing leadership to a small team of Care Coordinators.

Situated in the South Western Sydney, Illawarra and Shoalhaven region, the team provide outreach support and intensive case management for people with co-occurring alcohol and other drug and complex psychosocial support needs who are seeking treatment for their alcohol and other drug use.

The support you provide will further enhance the care coordination and access for the clients to AOD treatment and other services.


Your responsibilities:


  • Lead staff to ensure high quality, evidence based practice and care work.
  • Oversee Intake and Assessment processes, including care requirements and risk management
  • Support the Manager in the development, implementation, maintenance and review of the service model in accordance with the relevant polices and procedures.
  • Line manage and support Care Coordinators including professional development and performance plans, appropriate supervision, evidence informed practice
  • Ensure client data collection as required by the Society, regulators and funders
  • Develop, promote and implement referral pathways and facilitate access to range of community based health and nonhealth service providers
  • Support Care Coordinators with developing treatment and care plans that incorporate a holistic approach, with focus areas of alcohol and other drug treatment, employment, training/education, daily living skills, homelessness/housing, social engagement and participation, health, family and community reconnection and culture.

To be successful in this role, you will need:

  • Relevant tertiary qualification/s or VET sector and significant relevant experience.
  • Demonstrated extensive experience in managing a case load and case planning, including with persons with complex needs (e.g. cooccurring alcohol and other drug dependencies along with homelessness, mental health, contact with criminal justice system).
  • Demonstrated ability and/or capacity to manage and lead small groups of staff
  • Demonstrated ability to work therapeutically with clients (e.g. provide therapeutic interventions).
  • Demonstrated high level networking and community care coordination skills, particularly in the relevant region.
  • Knowledge of, and an ability to access, a range of relevant community resources.
  • Computer literacy skills including experience in computerised data collection
  • NSW driver licence (unencumbered)
