As a Disability Support Mentor member of JDC Support, you will be responsible for embedding best practice with person-centred
active support and positive behaviour support for our client. You will play a crucial role alongside a second support staff member
in providing support to ensure his individual human rights are protected, and that JDC Support's commitment to a zero
tolerance framework for abuse and neglect is upheld.
You are experienced in the provision of support to people living with an intellectual disability in their own homes; you are a
skilled Disability Support Worker someone who practices disability support with high complex individuals who require additional
support to lead an ordinary life. You are strongly committed and relish the opportunity to make a significant difference to the
life of this young man with the assistance of his support team. This role provides just such an opportunity.

Contract term:
To join the residential support pool of staff


Position purpose:
The Residential Support Mentor is responsible for assisting with the delivery of active
support within a house environment, to ensure the house runs smoothly and all dedicated
jobs within the household are done in alignment with JDC Support policy and procedures.
The Residential Support Mentor contributes to the embedding of person-centred active
support and positive behaviour support practice across the model of support being provided
to our client to the satisfaction of the Management of JDC Support. Full training provided.
The Residential Support Mentor delivers the care and support our client needs for successful
independent living and that he receives the maximum dignity and respect in his own home
and in the community. Ensuring at all times his behaviours are managed in accordance with
his current positive behaviour support plan and recognition for his learning and development
continue into the evening shifts.
The Residential Support Mentor encourages and supports full person centered active
support so our client can participate in his daily living skills into the evening shift, and that he
has full ownership and choice and control over his chores and life while being supported by a
team that respect him and listen to him.


Responsibilities:
General responsibilities

Primary goal of all staff to achieve the goals as defined in his NDIS Plan
Day to day responsibilities

  • Personal care support
  • Toileting, showering, personal grooming, shaving, hair washing, personal hygiene as
required on a daily basis

  • Support in everyday independent living
  • Working with our client on his ADLs, including his diet and cooking experiences
  • Meal preparation
  • Preparing and eating a wellbalanced diet daily
Positive Behaviour Support

  • Support and deliver positive behaviour support in line with the client's positive
behaviour support plan with implementation of known strategies, while maintaining
human rights, respect and dignity at all times

  • Incident recording in accordance with the company incident reporting system
  • Maintain relationship with management at all times for review and ongoing
discussions
Occupational Health, Safety and Wellbeing (OHS&W)

  • Take care for the clients health, safety and wellbeing
  • Follow OHS&W policies and procedures

Key selection criteria:


  • Prior experience working in a residential home environment essential
  • Demonstrated ability
a. In the delivery of positive behaviour support and implementation of behaviour
support plans and strategies
b. working with clients with an intellectual disability who have complex
behaviours and medical requirements
c. to work within a 2:1 team framework and with our client's extended care team
d. recognition and implementation of full person centred active support for
continued learning and development assisting him to achieve his NDIS goals of
independence and ADL skill development
e. Maintaining daily recording records and data collection records on computer

  • Relevant Autism experience minimum 2 years' experience required
  • Excellent working knowledge of the Disability Act 2006 (Vic)
  • Strong written and verbal communication skills clear c
