As the Aged Care Assessment Team Clinical Coordinator, you will work as a member of an interdisciplinary team and will be responsible for the coordination and leadership of the ACAT triage and clinical assessment service.

This position will maintain a 0.4FTE caseload involving more complex assessments and provide clinical support to members of the assessment team and contribute clinical expertise to achieve optimal patient and service outcomes.

The Aged Care Assessment Team (ACAT) conducts assessments to determine a client's eligibility for Commonwealth Aged Care Services. Assessments can be conducted within the hospital environment or within the individuals own home.

To be successful in this role, you will need:

  • Demonstrated leadership, role modelling, planning and management within a service that supports community, acute and the age care setting.
- at least 3 years, full time equivalent, post registration experience

  • Statement of Attainment in My Aged Care
  • Advanced Assessment Practice (SOA3/SOA4)
  • My Aged Care Self-Paced Learning Experience (SPLE)
This is a single vacancy, with two possible appointment classifications. The successful applicants will be appointed in the position appropriate to their skills, qualifications, and experience.
