MAIN DUTIES/RESPONSIBILITIES
- Great communication skills including; ability to build rapport, case noting, report writing and facilitation of care team meetings.
- Demonstrated ability to use NDConnect or other CRM software for daily updates of case notes and action items
- Writing and circulation of minutes of meetings to all stakeholders for clients
- Demonstrated ability to develop positive relationships with a range of participants, their families, and their service providers
- Awareness of local community programs and services such as homelessness services, mental health supports, mainstream housing options, justice system.
- Report writing and gathering appropriate evidence to advocate for adequate funding that meets your participants needs. Writing Plan Review reports and Change of Circumstances report, Plan Variation report and ability to communicate and speak with NDIS staff and planners
- Communicating with different service providers (e.g., Speech pathologist, VACCA, Office of the Public Advocate Guardians, Behaviour Support Practitioners, Supports Workers, Community Programs, DFFH, Justice Workers)
- Understand and adhere to appropriate policies and procedure
- Promoting the professional image of the organisation to internal and external stakeholders
- Assisting the Senior Support Coordinator in NDIS plan implementation
- Meet set KPI's
- Promoting independent decision making
- Work within the principles of personcentred practice
QUALIFICATIONS, SKILLS AND EXPERIENCE
Qualifications:
- Diploma or higher in Disability/ Mental Heath/ Social Work/ Psychology or other related studies (preferred)
- Current and Valid Police Check or willing to obtain
- Working Children Check
- NDIS Workers Clearance Check
- Valid Australian Drivers License
Experience:
- Previous support coordinator experience highly desired
- NDIS awareness, particularly in relation to support coordination and the importance of choice and control for participants
Skills:
- Knowledge and understanding of government disability services
- Capacity to complete all required documentation and task in a timely manner
- Ability to deal with changing situations of clients and prioritise work accordingly
- Ability to build rapport with a diverse range of clients
- Experience working with clients with complex support needs preferred
- Excellent written and verbal communication skills
