You need excellent communication skills both written and verbal, compassion, and empathy for others, with a natural ability to adapt to the client's style.

The Role:


The role is engaging and dynamic, requiring you to have a customer service client-centered approach as you are the focal point of contact for St Louis Clients.

The role requires proficient case management skills that promote choice, well-being, and independence for your clients; you will work with your clients to develop support and care plans and manage individual budgets.

The role requires ongoing assessment, reviews, and coordination of client choices and changing needs and requirements.


As Home Care Cooridinator you will ensure effective and efficient delivery of flexible, individually tailored Home Care packages to people who fall within the eligibility criteria of the Home Care Package Program, and ensuring compliance within the Aged Care Quality Standards.


Role requirements:


  • Tertiary qualification in health, community, aged services, nursing, social/human services, or equivalent industry experience and education
  • Working knowledge of the My Aged Care processes, and home care funding streams for home care packages.
  • Sound understanding of the Aged Care Quality Standards
  • Excellent decisionmaking and problemsolving skills to manage the provision of quality services, whilst balancing the choices of clients, staff, and the organisations you work with.
  • Strong interpersonal skills with a continuous improvement approach
  • Excellent verbal and written communication with the ability to problem solve, negotiate, and mediate
  • Understanding of casework practices for the aged, and people with complex needs including dementia
  • COVID vaccinations and boosters and and ongoing Influenza immunisation
  • National Police Clearance (Aged Care) with nil disclosable history
  • Unrestricted SA motor vehicle driver's license
  • Proficient IT Skills using Microsoft 365 suite, and other databases essiential
  • A knowledge of CIM is highly desirable
