The exciting opportunity:

5 day per fortnight with your choice of hours and days
100 bed residential aged care facility in the Rockingham area
Conduct thorough assessments and regular reviews to identify residents' physical, psychosocial, and emotional requirements
Create care plans that are aligned with resident needs and therapeutic goals, and prescribe any necessary equipment
Collaborate with the Lifestyle Coordinator to develop, implement, monitor and review meaningful programs that enhance independence and quality of life
Direct and oversee and Therapy and Leisure Partners to ensure appropriate care is being delivered
Cultivate reliable relationships with residents and address family concerns, promoting collaboration and effective communication

About you:

Current AHPRA Registration as Occupational Therapist
Exceptional interpersonal and communication abilities for establishing rapport with residents and families
Proven proficiency in conducting comprehensive assessments and developing personalised care plans
Enthusiastic, passionate, and confident in utilising outstanding clinical skills to deliver holistic care
