Description

The Support Coordinators duties include:

  • Understanding the participants NDIS plan, goals and objectives.
  • Meeting with and engaging the participant to develop and refine their goals.
  • Engaging participants informal support networks to build and strengthen them.
  • Engaging and coordinating service providers on behalf of participants and linking them to these service providers.
- assisting to implement the participants plan and increasing their capacity to manage/direct their own supports.

  • Assessing and selecting a number of mainstream, community, informal and provider options to increase the participants opportunities to explore and connect with their community and alternative support options.
  • Maximising the value for money they receive from their supports by negotiating services and prices.
  • Developing service agreements and create service bookings with preferred providers.
  • Arranging for any assessments required to determine the nature and type of funding required (e.g. assessment to determine the type of complex home modifications required).
  • Coaching to assist in independent accessing and use of the NDIS participant portal.
  • Preparing and submitting all reports required by the NDIS.
  • Identifying any needs or errors in NDIS plans and accordingly request appropriate NDIS reviews.
  • Provide connection and coordination and support participants to both establish new services under NDIS and transition from old services to NDIS.
  • Remain aware of and adhere to the conflict of interest policy for self, seeking advice from the Support Services Manager when required. Compliance.
  • Comply with Acts Care Policies and Procedures as applicable to your role, ensuring staff are accountable to the same standard.
  • Remain compliant with all mandatory training and participate in meetings and training as requested/required.
  • Ensure the NDIS Practice Standards are met at all times and mandatory reporting is completed as identified or instructed.
  • Keep accurate and complete records of your work activities in accordance with legislative requirements including department's records, information security, confidentiality and privacy policies

Salary:
$65, $80,000.00 per year
