About the role:

To support the Clinical Funding Resource Manager in optimising clinical funding claims through monitoring of clinical assessment.

Responsible for supporting the Care Homes to achieve funding outcomes by working with internal and external stakeholders to ensure congruency between the care provided to our residents, the residents assessments, care plans and the funding received.


In this role, you will have the opportunity to:

  • Partner with and support the home with the effective completion of resident documentation and charting.
  • Where feasible engage with and monitor resident's and their care needs with the view towards monitoring
- instilling effective processes within the home to identify and manage changes in care needs

  • Audit and evaluate assessments prior to the external AN-ACC assessors visit to ensure congruency and
- accuracy and that the care needs of the residents are reflected.

  • Mentor and coach care Home Leadership Team in effectively managing their assessments on admission
- in preparation for external AN-ACC assessors visit.

  • Work collectively with the General Managers, Clinical Care Managers to ensure care is delivered in
- accordance with assessed care needs and Resident Care Plans

  • Develop and maintain open and regular communications and relationships with all members of staff
  • Work in conjunction with the Clinical Funding Resource Manager on the submission and monitoring of
- requests for reassessments.

  • Support and assist the home to ensure effective carer documentation.
  • Provide training and where feasible manage the training needs of the home in relation to documentation
- and assessments.

  • Liaises with external AN-ACC assessors, supports the care home to prepare for an AN-ACC assessment.
  • Ensuring documentation and assessments are completed and available to the assessors.
  • Perform any other task as directed by line manager

To be successful in this role, you should have:

  • Knowledge of the Australian National Aged Care Classification (AN-ACC) (Essential)
  • Tertiary qualifications in training and assessment (desirable)
  • Registration with the Australian Health Practitioner Regulation Agency (desirable)
  • Ability to provide regular ongoing support directly within our Homes on a regular basis
  • Mentoring and coaching skills
  • Experience in liaising with key regulatory bodies
  • Change management experience
  • Proven written and communication skills
  • Advanced Clinical Assessment Skills
