The opportunity


RDNS, as part of the Silverchain Group, are actively seeking an experienced Home Care Package Coordinator to join our team in Woodville on a full time fixed term basis through till January 2024.


Key responsibilities
This role involves a high variety of tasks, a constantly changing workload and is extremely rewarding


With a person-centred approach, you will support clients in the community using your highly developed customer service and interpersonal skills.

Engage with our clients, their carers and families to manage individual budgets, care plans and goals.

Encourage choice, control and independence, as you assess, review and manage all Home Care Package services, allowing our clients to continue living their best lives.


About you

  • 35 years' experience working within the community providing coordination of care to clients, preferable including
  • Cert III in Individual Support (or equivalent)
  • Aged Care, Disability or Home and Community Care
  • Knowledge of service provision, coordination, allocation and compliance with contractual obligations
  • Exceptional communication and interpersonal skills to develop effective relationships, promote services and resolve client issues
  • Excellent decisionmaking and problemsolving skills
  • Intermediate computer skills
