Purpose of Position


We are looking for a talented and compassionate Case Manager with at least 2 years' relevant experience to join our the team at our supportive and quality-focussed workplace.

As a Case Manager, you must be capable of building positive and effective partnerships with clients, their representatives, and other stakeholders including clinical experts, allied health professionals as well as support workers.


Responsibilities:


  • Demonstrated case management experience, preferably in HomeCare Packages
  • Strong relationshipbuilding skills, a passion for selfmanagement, and empathy for customer support.
  • Effective communication and customer service focus leading to positive outcomes.
  • Capacity to manage highvolume case management.
  • Passion for supporting people to live a full life at home, using a goaloriented approach in developing and updating support plans.
  • Monitor and review client progress and make adjustments to care plans as needed.

Requirements:


  • At least 2 years' experience in case management in the Health, Aged Care or Community Services sectors; OR a formal qualification (Certificate III/IV or Diploma) in Community Services or Counselling. AHPRA registration as an RN or Allied Health Professional will be considered
  • Experience working within or demonstrated understanding of the Aged Care Quality Standards framework.
  • A National Criminal History Check / Police Check showing no disclosable court outcomes or convictions, and issued within 3 years.
  • A current, valid Australian Driver Licence.

Desirable:

  • Tertiary qualification in Social Work, Aged Care, Disability Services, Community Services Coordination, Nursing or Allied Health discipline with a current AHPRA registration, or equivalent experience.
  • Experience working in case management in either Home Care Packages or Community Home Support Program.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Spanish speaking.
