About the role

  • This position involves the provision of social work palliative care services for the
_GPCanShare_ Project.

The GPCanShare project is the formation of a new partnership between Sydney Local Health District (SLHD) and The Central Eastern Sydney Primary Health Network (CESPHN) in the delivery of primary health care to assist General Practice (GPs), community cancer services and specialist cancer services to work collectively in providing care for patients with cancer (and cancer-palliative care) and their families/carers, and improve their experience of the cancer journey.


This Social work position provides leadership, coordinates and facilitates integration with palliative care teams across the SLHD to GPs of SLHD patients diagnosed with cancer, palliative care teams, community-based palliative care services, SLHD Residential Aged Care Facilities, and facility and community-based allied health.


We are looking for someone who

  • Holds a Bachelor Degree in Social Work or equivalent providing eligibility for membership of the Australian Association of Social Workers.
  • Can demonstrate advanced clinical knowledge, skill and significant clinical experience in the provision of allied health services in cancer and palliative care.
  • Has extensive experience and a demonstrated ability in building and maintaining professional and therapeutic relationships and working collaboratively with the multidisciplinary team, health professionals and key stakeholders through advanced written and verbal communication skills, interpersonal, negotiation and conflict resolution skills.

About working for SLHD
This is a Category A position.

To be employed by NSW Health you are required to provide evidence of vaccination to comply with the NSW Health Occupational Assessment, Screening & Vaccination against Specified Infectious Diseases Policy (PD2023_022).


All NSW Health workers are required to have completed a primary course (2 doses) of a Therapeutic Goods Administration (TGA) approved or recognised COVID-19 vaccine (except for the Janssen COVID-19 vaccine which is approved by the TGA as a single dose primary course). New applicants must have completed the vaccination course prior to commencement with NSW Health, or provide an Australian Immunisation Register (AIR) Immunisation History Statement certifying the worker cannot have any approved COVID-19 vaccines available in NSW. A NSW Health agency may require further information about the medical contraindication (including but not limited to an Immunisation Medical Exemption form - IM011 form).

Acceptable proof of vaccination is the Australian Immunisation Register (AIR) Immunisation History Statement or AIR COVID-19 Digital Certificate. Booster doses are highly recommended for all health care workers who have completed the primary course of COVID-19 vaccinations.
