Your New Role- Provide exemplary social work service across but not limited to the community therapy service, including psychosocial assessment, emotional support and counselling, and discharge planning.

Demonstrate excellent communication strategies with patients, relatives, staff, and relevant services in order to provide appropriate services and information.
Liaise with service providers and refer to relevant community services, Aged Care Assessment Service and Transition Care Program
Enjoy the leadership and support of senior clinicians (grade 3) and clinical lead (grade 4) as well as the CTS multidisciplinary team

About You- You have demonstrated health social work experience with a collaborative working style.

You have excellent communication skills and a passion for continued professional development for yourself and others.
You are a qualified Social Worker, eligible for membership with the Australian Association of Social Workers.
Grade 2 applicants must have at least 4 years' experience.
You value working with supportive and dedicated health professionals and multidisciplinary teams.
