About the Organisation


The City of Port Phillip is an inner-city council with around 1000 employees who deliver more than 100 programs and services to our residents, businesses and visitors.

These range from childcare and immunisations, to libraries, management of parks and open space, rubbish collection, road maintenance and more.

About the Role


This is a newly created role which will lead the social connection program which is an important component of the new "Village Model" which is Councils commitment to supporting people to age well in the City of Port Phillip.

This service supports older people to access opportunities to socialise and stay connected, minimising the impacts of social isolation and improving health and wellbeing outcomes.


Duties:


  • Deliver a responsive, dynamic, and contemporary, social connection program within the scope of the CHSP.
  • Identify opportunities to grow the program or address unmet need ensuring that the programs meet the changing needs and expectation of the community.
  • Ability to form key partnerships that will support the strengthening of the program.
  • Work in close collaboration with the Community connection team to ensure opportunities to connect older people into programs are maximised.

Skills and Experience

  • Tertiary qualifications in social services, health sciences or other relevant fields.
  • Demonstrated experience on developing, maintain and continuously improving programming to meet the changing needs of clients and the community.
  • Previous experience in delivery of services within a CHSP funding framework.
  • Highly developed communication ad interpersonal skills.
  • Demonstrated ability to lead, support and develop high performing teams.
  • Thorough understanding of the Aged Care Quality Standards.
