About the role


We are seeking a dedicated Youth Services Case Manager You will provide flexible and assertive case management response to clients with complex needs to those experiencing homelessness.

Additionally, you will demonstrate commitment to work alongside young people and their identified support needs within the Barwon region.

Responsibilities include

  • Deliver high quality case management to young people accessing youth services programs
  • Complete comprehensive intake and risk assessment for all young people
  • Assist young people with range of activities in preparing for independent living: meals preparation; life and living skills; health and hygiene; recreation and exercise
  • Individual case plans are developed in collaboration with clients and include a range of interventions that address the needs or goals of clients
  • Clients assisted to access support services and establish community supports
This is a fixed term full-time position, for 12 months, based in Geelong, VIC. Salary and conditions are in accordance with SCHADS Level 5 Award.

You will have

  • Tertiary qualification in Social Science, Welfare or related field with a degree qualification preferred
  • Relevant experience in a Social Service environment
  • A full Driver's Licence
  • A valid Employee Working With Children's Check
