An outstanding opportunity has presented itself for an experienced Care Manager to join our collaborative Residential Care Team at Onkaparinga Lodge in Huntfield Heights.


About the role


As a Care Manager with Southern Cross Care, you will work with the Residential Services Manager to implement a healthy and active ageing service delivery model, which ensures resident choice, independence, and quality of life are maintained at the highest level.

You will work with your colleagues and residents to develop and implement goal-oriented care plans.

You will lead, support and mentor the care team to ensure optimal resident outcomes, and will lead the early intervention working group to develop recovery and fitness pathways for residents.

About you


With a Bachelor of Nursing or equivalent, current AHPRA registration and an impressive reputation for person-centred service in aged care, you will also demonstrate high-level listening, communication, team building and time management skills.
