you passionate about making a positive impact in the lives of NDIS participants? Do you have experience in the disability sector and a client-focused approach? We are looking for a dedicated Support Coordinator to play an integral role in empowering NDIS participants on their journey towards greater independence and achieving their goals within the NDIS framework.


Responsibilities:


As a Support Coordinator, you will:

  • Assist NDIS participants in implementing their plans and increasing their capacity to manage and direct their support.
  • Strengthen participants' informal support networks and maximise the value of received support.
  • Collaborate with participants to identify, coordinate, and implement a comprehensive range of supports, including informal, mainstream, community, and funded services.
  • Support participants in exploring and connecting with their community and alternative support options.
  • Facilitate negotiations of services and prices and aid in selecting preferred options or providers.
  • Act as the primary point of contact for participants regarding servicerelated matters, complaints, significant changes, and plan reviews.
  • Develop crisis management plans and risk assessments with participants, families, and carers.

Qualifications/Requirements:


  • Current WWCC, Criminal Record Check, NDIS Screening Check, and First Aid Certificate.
  • A relevant degree in social work, psychology, disability studies, community services, or a related field.
  • Ability to understand the NDIS Price Guide, legislation, and rules, including reasonable and necessary support provisions.

Experience:


  • Previous experience in the disability sector, support work, case management, or related roles.
  • Experience in case management or coordination is valuable.

Skills:


  • Clientfocused approach tailored to individual participant needs.
  • Strong communication and empathetic skills for effective engagement with participants, families, and stakeholders.
  • Creative problemsolving abilities to address challenges in support plans.
  • Ability to collaborate with multidisciplinary teams and coordinate holistic support.
  • Familiarity with industry regulations, standards, and ethical practices.
  • Cultural sensitivity for working with diverse backgrounds.
  • Proficiency in relevant software for documentation and case management.
  • Crisis management skills for handling complex situations

  • Evidence of participant satisfaction with selected services and support plans.
  • Effective communication and engagement with participants, community services, and colleagues.
  • Initiative in ongoing selfdevelopment and participant support.
  • Collaboration in team settings and positive working relationships.
  • Consistency in meeting commitments, deadlines, and NDIS guidelines.
  • Creative problemsolving and innovative support planning.
  • Adherence to professional standards and ethical practices.
  • Strong client relations through empathy, responsiveness, and respect.

Salary:
$85, $110,000.00 per year
