Position Summary
The Monash Health Post Acute Care Team is excited to advertise a permanent position available for a Clinician (Allied Health Professional or Registered Nurse).
This is a full time position and presents an excellent opportunity for like minded Clinicians to join a dynamic team that strives for excellence in Service delivery to our clients.

Working with clients across the aged spectrum, Monash Health Community provides a continuum of care, supporting clients through all stages of non-acute care from prevention through to rehabilitation.
About the Role:
The role is integral?to ensure that referred clients for PAC are triaged, registered and allocated to clinicians in order to co-ordinate services.

About You:
You are experienced, flexible and dynamic Nursing or Allied Health Professional that is looking for a change in career.
We are a high performing team and ensure that our client's journey through Community Health is seamless and timely, it is therefore vital that we have a like-minded Clinician to continue this journey with us
You Are Also:

What You Need:
- Relevant Qualifications in Allied Health or Nursing Related Field
- Current Victorian Driver's Licence
- Demonstrated recent experience working in an acute or community setting
- Minimum 5 years post graduate experience
- AHPRA registration or registration with National Registration Authority
- Australian working rights
