About the role


An exciting opportunity is available to be a part of our care communities new Meaningful Life Model of Care in Ballarat or Edenhope.


The Partner is responsible for the implementation of meaningful engagement that gives daily purpose to our individual residents, bringing joy to all that we care for.

This will be delivered through meaningful activities and purposeful engagement that supports a holistic wellbeing.


Key Responsibilities

  • Implement meaningful engagement, across the care community, that include activities that reflect a personcentred model of care, in line with the Aged Care Quality Standards.
  • To deliver meaningful activities and personcentred engagement, that meets residents' goals, needs and preferences.
  • Be responsible with the direction of the Meaningful Life Coordinator, to complete, all meaningful life residents' assessments, care plans, resident iCare profiles and documentation.
  • Provide support to other team members, volunteers and students in the care community.
  • Ability to manage time effectively whilst following daily schedule, including the settingup and cleaning up after activities.
  • Attend regular meaningful life team meetings and care community meetings as directed by the coordinator.
  • Advise coordinator and assist with organising resources required to support the meaningful life program.
  • Build connections and partnerships within the care community and the wider community.
  • Assist, provide and support residents to attend regular scheduled bus outing and to connect with the community.
  • Assist residents and relatives with technology to facilitate communication via scheduled video calls and phone calls.
  • Respond in a timely manner to any complaints or concerns raised.
  • Review, document and respond appropriately to potential risks, incidents and near misses.
  • Maintain equipment used in the leisure and lifestyle programs and report any maintenance or damage to team leader/manager.

Skills and experience

  • Hold a Certificate III in Aged Care (or equivalent) or Certificate IV in Leisure and Health (or equivalent) or willingness to obtain.
  • A Qualification in dementia care is desirable.
  • Demonstrate an understanding of Aged Care Quality Standards.
  • Excellent communication skill with the ability to work as part of an interdisciplinary team, with strong computer and customer service skills.
  • Experience with the Aged Care software program iCare is desirable
  • Victorian Driver's Licence
