We are looking for an experienced and enthusiastic Case Manager role based at the Auspire Care office, Coburg VIC 3058 to manage a caseload of elderly clients.

• Appointment will be in accordance with the Social, Community, Home Care and Disability Services Industry Award, Level 4 PPT
• There is a threemonth probationary period for new employees.

• Development and implementation of highquality individual care plans
• Development of supportive relationships with professional staff, suppliers, and families to ensure positive outcomes.
• Ongoing communication with clients and family members to foster partnerships and ensure choice and control of their individual care plans.
• Compliance with internal policies and procedures, statutory and contractual requirements in relation to the management of service delivery

• Diploma or equivalent qualification in Community Services
• Demonstrated experience in Case managing.
• Registered Nurse desirable
• Experience in implementing individual care plans, health, and wellbeing clients.
