What you'll be doing:

Nepean Hospital is currently recruiting a Level 1 or 2 Social Worker to work in Aged Care based conveniently at our Nepean Campus.

Our successful Social Worker will be responsible for providing a high quality social work service to the Aged Care wards at Nepean Hospital, and will require a working knowledge of social work theories which inform your practice.

You will also be required to work closely with the multiple disciplinary teams (MDT) on the ward/s to achieve best outcomes for our patients in an efficient and effective way.

Members of our Age Care Social Work team are required to complete psychosocial assessments, attend case conferences, facilitate family meetings, provide varying types of counselling and contribute to case and discharge planning.

All NSW Health workers are required to have completed a primary course of a COVID-19 vaccine which has been approved or recognised by the Therapeutics Goods Administration (TGA). Additionally, Category A workers are required to receive a booster dose three months after completing the primary course of COVID-19 vaccinations. New applicants must have completed the vaccination course prior to commencement with NSW Health, or provide an approved medical contraindication certificate (IM011 immunisation medical exemption form) certifying the worker cannot have any approved COVID-19 vaccines available in NSW.

Acceptable proof of vaccination is the Australian Immunisation Register (AIR) Immunisation History Statement or AIR COVID-19 Digital Certificate. Booster doses are highly recommended for all health care workers who have completed the primary course of COVID-19 vaccinations.

Employment of a temporary visa holder may only occur if no suitable permanent resident or citizen of Australia has been identified and must not exceed the duration or conditions associated with the current visa.

Selection criteria to be addressed:

Tertiary Qualification in Social Work and eligibility for membership with the Australian Association of Social Workers (AASW)
Demonstrated understanding of Aged Care Social Work within a Hospital environment
Demonstrated high level communication, report writing and computer skills
Demonstrated ability to work independently and as a part of a Multidisciplinary team.
Demonstrated understanding of psychosocial stressors that could impact on the admission.
Basic knowledge of My Aged Care, Carers Gateway, NSW Civil Administrative Tribunal
Guardianship Division, National Disability Insurance Scheme, the Ageing and Disability Commission and Residential Aged Care.
Demonstrated capacity to Link theory and practice
Current unrestricted driver's license and willingness to travel for work.
