About the role

Conduct frontline clinical duties as per the IUIH Model of Care for AHW.

It is the role of the AHW to contribute, where possible, to ATSICHS comprehensive health support services to clients with the aim to achieve improved health outcomes for all Aboriginal and Torres Strait Islander people.


This includes effective:
triage, prevention, education, support and recalls.

Duties

  • Provide quality Aboriginal health worker practice within an Aboriginal primary health care model of service to implement and support a medical/primary health care team and effectively plan, and coordinate own and team's workload to achieve results.
  • Understanding of the principles of primary health care and the ability to work with a wide variety of professionals and the public, as part of a team.
  • Effective communication skills written, verbal and interpersonal to ensure medical documentation is of quality.
  • Commit to a high level of quality management for ATSICHS through use of LogiQC quality management systems.
  • Demonstrated clinic experience, in particular the ability to implement and support a medical/primary health care team and effectively plan, and coordinate own and teams workload to achieve results.
  • Commit to ongoing continuous quality improvement processes with annual reviews into the efficiency of clinical service provision.
  • Cultivate productive working relationships at all levels, which support the nurturing of internal and external relationships, facilitates cooperation, trust and respect, values differences and diversity and guides, mentors and focuses on the development of staff.

Skills and Experiences:


Essential:

Certificate III in Aboriginal Primary Health Care (Clinical Practice)
Previous demonstrable experience in relevant Aboriginal health worker field.
Demonstrated ability to meet targets and performance outcomes.
Ability to support a continuous improvement model for achieving outcomes.
Decisive listening, influencing and persuasive skills.
Working with Children's Card (Blue Card) or be eligible to apply


Desirable, but not mandatory:

Minimum two (2) years' experience in a Primary Health setting.
Previous experience of working with Aboriginal and Torres Strait Islander clients.
Previous experience in a multidisciplinary health team.
Previous experience in chronic disease management.
