We have a fantastic opportunity for an experienced and dedicated Health and Wellness Assistant to join our collaborative Health and Wellness Team supporting our Northern and North Eastern Suburbs community.


About the role


As a Health and Wellness Assistant with Southern Cross Care, you will help older Australians to live better for life, in their own homes.


Key Responsibilities include but are not limited to:

  • facilitating exercise program designed by an Allied Health Professional
- engaging with clients in a meaningful capacity promoting best health pathways
- personal care as well as domestic support; maintenance of the client's environment, assisting with shopping etc.
- following healthy aging principles and SCC's Better for Life model in all interactions with clients
- listening, with empathy, to the client and their representatives about the client's needs, goals and preferences
- keeping concise and accurate records of care and services provided and the client's response
- assisting with other duties such as deliveries, online training, administration and other office assistance as directed by the Program clinical Lead

About you
We are looking for people who will be the difference in the lives of our residents and clients.

  • Certificate III in Individual Support (Ageing) or Certificate III in Aged Care or a Certificate III in Disability or other related discipline
  • Previous experience in a personal carer role without direct supervision
  • Sound best practice personal care skills appropriate to the role
  • Current and unrestricted driver's licence & own registered and insured car
  • Good understanding of the requirements to support clients living with dementia including those who display challenging behaviours
  • Strong organisational and time management skills
  • Ability to build positive relationships with clients and their representatives
  • Basic cooking skills
  • Senior First aid certificate desirable
  • A basic understanding of aged care packages desirable
