About Aged Persons Mental Health Service
The community teams have seven sub-teams based in Kingston Centre and Endeavour Hills.

The service provides assessment and treatment for people aged 65 years and over, who have a psychiatric disability and/or dementia with associated behaviours of concern requiring specialist treatment.

From each site services such as the Aged Psychiatry Assessment Team (APAT), the Residential Support Program (RSP), Aged Persons Intensive Community Treatment (ICT), the Psychogeriatric Nursing Home In-Reach (PGNH In-Reach) team and Aged Persons Triage are provided.


About the Role
The service comprises of a team of Mental Health Clinicians, including RPN's, OT's and Social Workers.

The Mental Health Clinicians are supported by Consultant Psychiatrists, Registrars, Neuropsychologist, a Primary Care Transition Coordinator, Administrative support and this role.


This position plays a dual role of providing leadership and support in the care of consumers who are prescribed Clozapine as well as towards improving physical health outcomes of consumers of the Aged Persons Mental Health Service.

As a Clozapine and Physical Health Clinician you will work collaboratively with other clinicians to ensure quality of care is provided to consumers in the service area.

We will look to you to provide sound clinical input to the team and demonstrate competence in supporting, enhancing and facilitating the care of consumers.

As this is a Grade 4 nursing role sound leadership, networking and collaboration are essential traits that will allow you to perform your best in this position.

Your ambitious and supportive disposition will see the success of the team reach many goals such as, improved physical health outcomes of consumers and adherence to all Clozapine protocols and standards.

Your approach is patient centred and view exceptional outcomes as a top priority.

In clinical matters you will demonstrate your excellent knowledge base in dealing with complexities as well as being an effective resource for the team regarding Clozapine and physical health.


We are looking for someone with:

  • Minimum 5 years as a Grade 3 Registered Psychiatric Nurse
- great communicator and capacity to network and collaborate

  • Patient centred
  • Experience with Aged Mental Health desirable but not fundamental
- excellent networking and collaboration skills
