We are seeking an experienced professional with knowledge and understanding of financial processes and guidelines related to individual funding and budgets.

Working closely with our Home Care Services team, you will be responsible for developing and coordinating care for customers on a day-to-day basis.

This involves engaging and collaborating with the Hub team, our customers, their families and/or representatives. This position will facilitate the assessment, planning and delivery of services for customers, emphasizing person-centered care. The Assistant Hub Manager will maintain consistency of service delivery and overall contractual, regulatory, and legislative compliance.

Reporting to the Hub Manager, the position serves as the primary contact point for customers and their families regarding services delivered.


Duties:


  • Facilitates appropriate services for customers according to their service requirements
  • In conjunction with the Hub Team, provides effective leadership and guidance to support staff, in order to deliver high quality services that meet the customer's goals of care
  • Shares expertise and experience with other Hub Team members, through active networking and stakeholder engagement
  • Establishes and maintains effective communication with customers and families to ensure they are kept informed of any service changes and or developments, as appropriate
  • Manages the approved individual budget linked to customers
  • Actively participate in a culture of continuous improvement and will be required to meet deadlines, manage competing priorities and be solutions focused.
  • Ensures documentation used as part of the consumers care is maintained and encourages input into consumers progress notes from GP, allied health professionals and any other appropriate members of the Hub Team
  • Ensures safe and sound work practices that comply with work, health and safety policies and procedures and relevant legislation

Skills and Experience:


  • Relevant tertiary qualifications and/or relevant industry experience
  • Solutions focused high level conceptual and analytical skills
  • Highly developed communication skills (both oral and written) and interpersonal skills
  • Knowledge of financial processes and guidelines related to individual funding and budgets
  • Sound organisational skills with the ability to work independently, meet deadlines and manage competing priorities

  • Valid Drivers Licence
  • Ability and willingness to travel throughout the state
