Social Worker - Disability Practitioner

Employment Term - Ongoing - Full Time or Part Time (minimum 3 days per week)

Total Remuneration Package - $84,336 - $113,819 includes Annual Leave Loading & Superannuation + Car Options + Mobile + Laptop

If this sounds like you then Everyday Independence is ready to train you to become a Disability Practitioner working in an interdisciplinary team to better the lives of our community.


This is a newly created role and is part of our contemporary and innovative team of practitioners who meet regularly to celebrate successes, unpack challenges and to learn.

In this role you will be using our new LEAD TOOL to undertake initial assessments to set participants up for success on their journey with us.


As a Disability Practitioner, you will deliver best-practice, person-centered assessment, planning and support to people and their families living with a disability.

You will build relationships with people and their support networks as you help them to create a vision for a life without restrictions and with positive interactions, activities and experiences.

Working with people of all ages and achieving outcomes for greater quality of life.


As a Disability Practitioner, you will:

  • Play a vital role in helping people to enjoy everyday life by understanding what is important to them and how best to support them.
  • Champion the role of a social model practitioner and support people to overcome the barriers that are holding them back from engaging with their local community and the wider world. And you won't do this alone. With the backing of an interdisciplinary team and Habit Coaches, you'll be able to wrap the right supports around participants, empowering them to achieve greater outcomes.
  • Use evidence based practice and professional reasoning when recommending the most appropriate team of supports, therapy, duration, location, and timing.
  • Develop effective and strong working alliances with people and their families to identify factors affecting a working alliance, and repair breakdowns in a timely way.
  • Enhance the capacity of people to make their own life choices and increase their independence.
  • Approach risk in a positive way to achieve quality of life outcomes, whilst ensuring that safeguards are in place to maximise the rights of people and protect the most vulnerable.

Your Everyday
If you were here last week these are some of the things you may have done:

  • Undertaken 5 or 6 initial assessments using the LEAD tool and liaised with the practitioner team to wrap the right supports around the participant and their family.
  • Supported a person in their natural environment such as their day program or home to bring the person's hopes and dreams to life.
  • Attended peer group supervision/mentoring and internal supports to help grow your expertise and skills
  • Completed online learning through our LMS system.
  • Participated in a weekly team meeting and contributed thoughts, ideas and solutions
  • Completed reflective practice to develop evidencebased plans
  • Worked collaboratively within an interdisciplinary team
  • Shared a joke and a laugh with colleagues and maybe attended a hub social event

To be successful in this role, you will:

  • Qualification in Social Work and full membership of the Australian Association of Social Workers
  • Minimum 12 months experience working as a social worker
