The role you could play


We are seeking a dedicated Case Manager for our Youth Services You will be required to provide assertive case management response to clients with complex needs.

Additionally, you will ensure clients in the North West region receive assistance for the transition out if the homelessness system.

Duties

  • Deliver high quality case management to young people accessing youth services programs
  • Complete intake and risk assessment for all young people, incorporating adequate assessment of needs of accompanying children where relevant
  • Develop case management plans in assisting young people to attend appointments
  • Assist young people with a range of activities in preparing for independent living
  • Individual case plans developed in collaboration with clients that include a range of interventions that address clients needs/goals
This is a max-term full-time position, until August 2024. This role is based in Sunshine, VIC. Salary and conditions are in accordance with SCHADS Level 5 Award.

  • Tertiary qualification in Social Sciences, Welfare or related field with a degree qualification preferred
  • Relevant experience in a social service environment
  • Current Victorian Driver's Licence
