Due to an internal promotion the deafblind services team is looking for a passionate Support Coordinator to join their team and make a meaningful impact in the lives of those that we support.

National Office in Surrey Hills, VIC (with 50% WFH options and some travel required within the state to meet with clients).

SCHADS Level 3
(plus an hourly Auslan Bonus)


As the Support Coordinator you will assist NDIS clients in the support co-ordination and implementation of all supports in their funded plan, including informal, mainstream and community supports, as well as funded supports.


_ Responsibilities for Support Coordination:
_


Undertake a mixed portfolio to ensure service recipients:

  • Pursue their goals, objectives and aspirations
  • Increase their independence
  • Increase social and economic participation, and build their capacity to actively take part in the community

Skills and experience required to be successful in this role:


_ Essential:
_


  • Experience and skills to work competently alongside people, their family and carers in their local community to build their capacity to participate in the community and manage some or all aspects of their plan.
  • An understanding of support coordination practices and principles.
  • Knowledge of the current trends in service delivery in the community sector, particularly the changing landscape of the disability sector.
  • Welldeveloped interpersonal and communication skills with the capacity to liaise effectively with a wide range of clients and service providers, including deafblind clients.
  • Demonstrated ability to identify, measure and report on outcomes.
  • Capacity to prepare reports and correspondence in clear and concise language and the ability to assimilate information from other, present information in a manner appropriate to the relevant parties.
  • Competent in the use of the latest computer software programs and is able to enter, modify and extract data accurately, including the ability to track funding package expenditure.
  • Knowledge of the _NDIS Act 2013_, _Disability Act 2006 _and other relevant legislation.
  • A tertiary qualification in a relevant discipline.

Auslan skills highly regarded, hourly bonus will be added to salary if you have qualifications, or are fluent in Auslan (Including CODA)

  • Understanding of those living with disabilities including: working with people living with a disability, lived experience with family members, experience working with people with deafblindness.
  • Knowledge of the NDIS Operational Guidelines.
  • Experience working in an NDIS environment.
  • Flexible to work from other sites.
