Our mandate is to seek social justice, alleviate social inequality and enable better healthcare for people seeking asylum and people experiencing disadvantage within Australia and overseas.
The Asylum Seeker Health Program is based at the Cabrini Asylum Seeker and Refugee Health Hub in Brunswick.Our primary target groups are people seeking asylum who are Medicare-ineligible and those who have Medicare but limited or no income.
Our service provides integrated primary care, mental health care and health waiver programs, and is a recognised leader in the work we do winning the Victorian Multicultural Commission Awards for Excellence in Health in 2021.
The innovative Primary Care Service is a nurse-led model (combining practice nurse/refugee health nurse) working alongside a mix of pro bono and employed general practitioners and the Medical Director.
The team works closely in an integrated model with the Specialist Mental Service and other external community-based organisations supporting people seeking asylum.
The Role
The Team Leader - Primary Care/Refugee Health is a key member of Cabrini Outreach's Asylum Seeker Health Services responsible for the supervision and leadership of nursing staff in the primary care teams, as well as a clinical role working as a practice nurse/refugee health nurse.
Key responsibilities:
- Overseeing the day to day clinic management, contributing to data collection and reporting, ensuring adherence to policies and procedures, participating in management meetings and sector engagement.
- Supervision and leadership of direct reports
- Direct clinical care of clients through assessment, care coordination and management of clients such as triaging new referrals, undertaking comprehensive clinical assessment, developing treatment and management plans, direct clinical care, health education, coordination of care, appropriate internal referrals, as well as facilitating external referrals to other health and community services when required.
We are seeking a Registered Nurse with at least five years clinical experience who has been a team leader, ANUM or held another healthcare management role, as well as someone who is passionate about working with people seeking asylum in a primary care setting.
You will have:
- Current Australian Health Practitioners Regulation Agency registration as a Registered Nurse.
- Nurse Immuniser (highly desirable).
- Previous experience working with asylum seekers and refugees (highly desirable).
- Ability to provide and be responsible for supervision and leadership of the primary care team, including the ability to delegate and to motivate the team.
- Ability to work autonomously under limited supervision, exercising sound professional judgement and seeking advice and consultation when appropriate.
- Experience in undertaking triage, assessments, developing plans, providing health management information and complex care coordination.
