About the facility


Barwon Health Palliative Care provides palliative care nursing, medical, allied health, supportive care and volunteer services within a framework of integrated health service integrating community, inpatient as well as an acute consultative service.

The model of palliative care is based on level of need and targets patients with complex care needs


About the role


In this role you will provide advanced and professional supportive care to people who are living with a life limiting illness, and their families and carers, within a supportive care context.

This position is based within the Supportive Care team of the Palliative Care Program and provides service to people receiving care from the Palliative Care program team.

The position has a strong focus on providing therapeutic counselling from a psychological, social work or grief and bereavement professional perspective.
