Main duties and responsibilities:


The Case Manager is key in providing the provision of person-centered disability services and is central to develop the capacity of the organisation and its staff to manage and support participants with a wide range of disabilities.

The role is directly responsible for understanding and embedding the quality standards pertaining to the provision of disability services within the NDIS framework.

The Case Manager is to provide ongoing coaching and upskilling opportunities to staff to deliver intended outcomes for participants with a disability to foster and enhance quality of life and care individualised to person.


Personal & professional capabilities required:


  • Willingness to promote Allambi Care's (AC) Needs Based Restorative Framework with a key focus on Trauma Informed Practice and Individually Assessed Needs.
  • Ability to provide leadership, direction and supervision to Case Work Managers and Disability Support Workers
  • Capacity to provide strategic direction and interpreting of the goals of the service in daytoday functions and tasks
  • Ability to participate in the development of longterm strategic goals for departmental programs
  • Ability to collaborate with external agencies and internal AC programs Support Case Work Managers in developing Individual Service Packages (ISPs) and Service Agreements for participants and ensure these documents are updated as required.
  • Capacity to provide support, direction and encouragement to Case Work Managers when assisting NDIS participants in reaching their full potential/achieving their individualised NDIS Goals.
  • Provide a professional approach, have effective communication and team building skills to enable the team to provide high quality support to participants.
  • Take an interactive and dynamic case management approach, with an emphasis on ongoing analysis, decisionmaking and record keeping ensuring the participant's needs are identified and met
  • Provide effective management in the assessment, planning, implementation, monitoring and review of plans to achieve identified outcomes for Participants
  • Work in collaboration with the other internal/external services to ensure service delivery is integrated, coordinated and tailored to meet a Participant's needs ongoing
  • Develop strong relationships with external stakeholders, including the extended family and significant people in the Participant's support network to enhance and foster quality of life.
  • Ensure team members are complying with Allambi Care's reporting requirements (i.e incident reports, restrictive practices, FBAs, DCATS etc)
  • Be responsible for Regulated Restrictive Practices processing, reporting and implementation.

Key Areas of Work:


  • Ensure all Restricted Practices and their uses, whether authorised or unauthorised are reported in a timely manner through the RPA Portal and PRODA
  • Work to allocated budgets and manage budgets throughout the year for individual participants
  • Understand and work within guidelines and requirements of different participant funding streams such as the National Disability and Insurance Scheme (NDIS), NSW Trustee & Guardian, and selfmanaged accounts.
  • Monitor usage and allocation of supports according to participant funding levels and goals
  • Prepare responses to Official Community Visitor (OCV) Reports and submit them in a timely manner
  • Ensure all participant review meetings are held at appropriate intervals as outlined by the NDIS, Office of the Children's Guardian (OCG) and Disability Service Standards (DSS)
  • Ensure all reports are sent to NDIS, Department of Communities and Justice (DCJ) or other peak bodies in a timely manner
  • Support and assist in implementation of appropriate behaviour management strategies and ensure quality reporting/documentation and follow up regarding incidents.
  • Collaborate with the Office of the Ombudsman and to ensure all legal requirements are met
  • Participate in the development and review of departmental program policies and organisational policies
  • Ensure all AC policies and procedures are followed and communicated to Case Work Managers
  • Ensure any changes to AC policies and procedures are communicated and trained to Case Work Managers
  • Ensure all standards are met in accordance with NDIS Quality and Safeguards Commission (NDIS Q&S), Voluntary Out of Home Care (VOOHC), and the DSS
  • Coordinate and attend weekly departmental meetings
  • Coordinate regular employee support meetings and provide support, feedback and training to Case Work Managers
  • Ensure participant files are maintained, accurate, uptodate and in line with DSS standards, NDIS Q&S standards and NDIA standards
  • Attend critical incidents within office hours and ensure on call staff are provided with accurate and detailed information to support staff and participants after hours
  • Ensure debriefing of all involved staff immediately after a critical incident and debriefing of staff follow
