- Monday to Friday, on a 3-month assignment with potential for extension
- Conduct comprehensive home-based clinical OT assessments and interventions for older individuals.
- Assess occupational performance and participation, functional levels, and environmental suitability for assistive equipment.
- Develop professional, clear, and concise clinical documentation and routine reports to organisational standards.
- Provide expert assessment and prescription of home modifications and assistive equipment tailored to individual needs.
- Work autonomously and collaboratively to deliver high-quality Occupational Therapy services within the community.
- Minimum 2 years of experience in Community Care, with a strong background in Occupational Therapy, especially within aged care.
- Demonstrated proficiency in manual handling and the provision of community-based Occupational Therapy services.
- Current National Police Check and Working with Children Check.
- Valid Driver's Licence and access to reliable transportation.
- Level 2 First Aid Certificate.
- Excellent communication and documentation skills, with the ability to work both independently and as part of a team.
