    Description

    This Occupational Therapist - Aged Care position within a Local Council in Wantirna South is dedicated to supporting the elderly in the community, focusing on home-based assessments, interventions, and ensuring their independence.

    Key Responsibilities:

    • Conduct comprehensive home-based clinical OT assessments and interventions for older individuals.
    • Assess occupational performance and participation, functional levels, and environmental suitability for assistive equipment.
    • Develop professional, clear, and concise clinical documentation and routine reports to organisational standards.
    • Provide expert assessment and prescription of home modifications and assistive equipment tailored to individual needs.
    • Work autonomously and collaboratively to deliver high-quality Occupational Therapy services within the community.

    About You:

    • Minimum 2 years of experience in Community Care, with a strong background in Occupational Therapy, especially within aged care.
    • Demonstrated proficiency in manual handling and the provision of community-based Occupational Therapy services.
    • Current National Police Check and Working with Children Check.
    • Valid Driver's Licence and access to reliable transportation.
    • Level 2 First Aid Certificate.
    • Excellent communication and documentation skills, with the ability to work both independently and as part of a team.
