    We are currently seeking an experienced Care Coordinator for a full-time ongoing role based in Bundall. As a vital member of our team, you will oversee and facilitate the provision of high-quality care and services to our clients.

    Key Responsibilities:

    • Perform client care assessments and oversee ongoing care planning
    • Manage client compliance and maintain a case load of clients
    • Coordinate the delivery of personalised care tailored to clients' needs and care plans while adhering to allocated funding and budget requirements
    • Engage in general administrative tasks associated with the role

    Desired Skills:

    • Prior experience in an aged care/NDIS/mental health care coordination role is advantageous
    • Familiarity with My Aged Care, Home Care Package, and NDIS systems
    • Experience in evaluating and planning care needs within the community
    • Proficiency in using computer systems, including Microsoft Office 365, Sharepoint, Outlook, and rostering software
    • Proven ability to manage incoming calls in a friendly and professional manner
    • Demonstrated capacity for planning, organisation, problem-solving, and solution-finding
    • Positive attitude with a strong aptitude for teamwork
    • Outstanding networking and consultation skills to establish relationships with external organisations for referrals
    • Exceptional customer service skills, both written and verbal, while maintaining confidentiality and privacy in accordance with company policies

    Qualifications

    Mandatory Credentials:

    • National Police Check (Compulsory)
    • Valid Australian Driver's License (Necessary)

    Additionally:

    • NDIS Workers Screen Check Clearance and Orientation Module Certificate
