  • Overview

    The Yorke and Northern Local Health Network (YNLHN) Healthy Ageing Team is responsible for providing Community Aged Care services for programs such as Commonwealth Home Support Program, Day Centre Programs and Aged Care Assessment Team. These programs must meet the Commonwealth Aged Care Standards delivering client centred, goal driven, reablement services to older people within communities across the Yorke Peninsula.

    The Lifestyle Programs Support Officer is responsible for the delivery of centre based activities for Client in the Commonwealth Home Support Program, Home Care package, NDIS and other associated programs.

    About the role

    The Lifestyle Programs Support Officer liaises with clients, families, carers, clients representatives, and other members of the multidisciplinary team to:

  • Provide social support to Aged Day Program participants in line with Aged Care Quality Standards, Commonwealth Home Support Program guidelines, NDIS guidelines and any other associated program guidelines
  • Plan, organise and provide diversional therapy for Aged Day Program participants in conjunction with the Day programs Coordinator
  • Promote independence and social participation with Clients
  • Eligibility

    We are looking for applicants who have completed a AQF Certificate relevant to Aged Care.

    Applicants must work well within a team environment, provide assistance and co-operation to other staff, be able to meet deadlines and timeframes, have a flexible approach to work and rostering, and have great interpersonal and communication skills and the ability to relate to people from different cultures, backgrounds and circumstances.

    Experience required:

  • providing a direct care service in a health related field
  • dealing with aged, frail or disabled persons
  • general office administration
  • working within a team environment
  • competent use of Microsoft Office packages (Word, Publisher, Outlook, Excel, Access and Power Point).
