    Description

    Position Summary

    An exciting opportunity exists for a passionate and patient-centred Grade 2 Occupational Therapist to join the Western Health at Home Division for a fixed term contract, supporting patients across our Service and Community Based Transition Care Program. These programs work closely together to ensure patients can access an alternative option to hospital-based rehabilitation and ongoing time limited care coordination by offering holistic interdisciplinary assessment and care for older people with complex, chronic or multiple health conditions within their own home environment rather than in hospital.

    The Western Health at Home Division aims to:

  • Improve the patient experience by allowing them to receive care in the comfort of their own home
  • Assist patients to achieve their care goals and best possible level of health and function
  • Promote and support safe and early discharge from inpatient settings
  • The successful applicant will work in partnership with a strongly committed team to shape the provision, service development and implementation of high quality and efficient care to patients in the home environment.

    The Western Health at Home Division is looking for OT candidates that:

  • Possess strong interpersonal and clinical skills
  • Work collaboratively within the inter-disciplinary team
  • Demonstrate critical thinking in practice
  • To be Successful in the Role, you must have:

  • Appropriate recognised tertiary qualification and Registration with the Australian Health Practitioner Regulation Agency (AHPRA) as eligible to practice as an Occupational Therapist.
  • Experience/qualification in aged care and/or community based occupational therapy
  • Highly developed interpersonal, written and electronic communication skills
  • Excellent organisational and time management skills
  • Commitment to continuing professional development
  • A current Victorian driver's licence
  • This position is offered on a temporary full- or part-time basis (job share options will be considered).
