    headspace provides support to young people aged 12-25 experiencing mild to moderate or emerging mental health issues. The multidisciplinary team at headspace Tuggeranong work to improve health, social, vocational and educational outcomes for young people.

    Grand Pacific Health is the lead agency of headspace Tuggeranong. We are a not-for-profit, primary health care organisation providing locally-tailored, high quality physical and mental health services across NSW and the ACT.

    About the position

    The Youth Care Coordinator will be based at our headspace Tuggeranong service and will work collaboratively with a young person and their family/carers to assess the mental health and psycho-social support needs of the young people referred to headspace.

    You will coordinate transition to appropriate support and treatment streams within and external to headspace in accordance with the clinical need. The position will also have a community engagement component, working closely with the community engagement officer for health promotion and related activities.

    The position will work closely with the other members of the headspace Tuggeranong team and external service providers to coordinate support needs.

    This position is part-time (3 days per week), fixed term until 30 June 2025 with the possibility of extension.

    We are looking for someone with:

    1. Qualification in or working towards the same in youth work, community welfare, social work, psychology, peer work, occupational therapy, counselling or other relevant field or demonstrated experience in a similar role.
    2. Demonstrated understanding of issues affecting the health and wellbeing needs of young people with mental health and/or substance use problems.
    3. Experience working within a team environment, providing coordination of care and ability to work from a recovery-oriented framework
    4. Passion and experience working with young people
