Northeast Health Wangaratta (NHW) is the major health provider in the Central Hume region with an extensive range of acute, sub-acute, aged care and community services.
We are seeking an experienced Registered Nurse or Allied Health Professional with a passion for collaborative patient care to join our Post Acute Care team as a clinical Care Coordinator.
The Care Coordinator - PAC provides comprehensive clinical assessment, needs identification and service coordination to our clients following an hospital stay. The role provides virtual care to clients, with face-to-face care delivered by brokered providers. The successful applicant will join a well-established and supportive team and will work closely with referrers and community service providers to ensure patients receive appropriate supports after hospitalisation.
