    About the Role

    You have been working for 2 years within Mental Health and enjoy supporting consumers to reach their mental health recovery goals. You are familiar with case management and have an interest in working with consumers who present with complex mental illness.
    The main duties you will perform in this role include the following:

    • Provide case management for consumers within the Hawthorn Clinic
    • Be familiar with completing risk assessments, developing treatment plans and other associated documents
    • Provide education and care to families, carers and supporters around a consumers mental health
    • Partner with other organisations to ensure holistic supports are provided to consumers.

    Classifications for this position will range from:

    • OT/SW Grade 2 YB20 - YB23 or YC42 - YC45 ($86,304 - $99,210 per annum)
    • Psychologist Grade 2 PK1 - PK4 ($90,812 - $102,757 per annum)
    • RPN Grade 3 NP81 - NP74 ($99,439 - $110,432 per annum)

    Your Contribution

    • A minimum of at least 2 years post qualification clinical experience within Mental Health for SW, OT and RPN
    • Registration with Australian Health Practitioner Regulation Agency (all disciplines except Social Work)
    • RPNs - Nursing and Midwifery Board of Australia (NMBA)
    • Relevant postgraduate qualification in mental health for nursing staff
    • Occupational Therapists - Registration with the Occupational Therapy Board of Australia
    • Social Workers - An approved degree in Social Work (by the AASW) and eligibility for full membership of the Australian Association of Social Workers.
    • Current Victorian driver's license
    • Current National criminal history check, or willing to obtain
