- Manage and implement disease management programs for members, to achieve HCF's objectives in line with the HCF, Member Health and Health and Wellbeing strategy
- Manage and partner with key internal stakeholders to assist them with their health and wellbeing needs.
- Monitor and report on effectiveness of the disease management programs
- Support Frontline staff with member enquiries regarding current disease management programs
- Recommend enhancements/expansion/rationalization to current programs to ensure uptake, effective disease management and risk identification
- Strong organisation and relationship management skills
- Excellent Project management knowledge and ability to use excel proficiently
- Excellent verbal and written communication skills
- Ability to streamline and manage processes
- Clear focus on the market in regard to member's needs and expectations
- Experience with health & wellbeing programs would be an advantage

You will ensure that HCF's Health and Wellbeing strategy is implemented to plan and is operationally efficient.
