Barwon MHDAS provides integrated and recovery oriented mental health, drugs and alcohol assessment, treatment and community support to people of all ages within the Barwon region experiencing, or at risk of experiencing, mental health and drug & alcohol issues, and their families.

About the facility
Barwon Health's Adult Mental Health Services provide community and in-patient services for people aged between 16 and 65 years, affected by serious complex mental illness.
Five community-based adult mental health teams provide services to the Barwon sub-region. The teams are multi-disciplinary and service a specific geographical area.

About the role
The role of Adult Community Case Manager is to provide assessment, treatment and recovery based interventions to individuals and their families who experience serious mental illness. The broader service delivery model includes provision of high quality interventions that are consumer-centred, family/carer inclusive and are delivered in a timely and coordinated manner in line with the Victorian Mental Health Act 2022.
Other assigned duties (portfolios) will be determined by the Continuing Care Services Manager, Senior clinician and/or Consultant Psychiatrist and are likely to incorporate one or more of the following:
