    Description

    ABOUT THE ROLE:

    You will be part of the Community Plus team which is like a hybrid of MST and HOPS teams. It's a recovery focused multi-disciplinary team who provides an extended hours clinical service to people who live within the municipalities of Wyndham, Hobson's Bay and Maribyrnong. In addition to experiencing significant mental illness/ mental health problems your clients will have other health and psycho social needs that impact their wellbeing Notably many of your clients will experience chronic homelessness. The role has a dual purpose. Firstly, to provide clinical case management to eligible clients with an emphasis on working with clients who experience homelessness. Secondly to support the housing and homelessness service sectors so they can optimally support people with significant mental health issues. You will facilitate comprehensive responses for your clients' needs and as senior clinician you will provide support and leadership within the team.

  • fixed term, fulltime RPN4 (80 hrs per fortnight with monthly ADO). Part time applicants will be considered

  • ABOUT YOU:

  • Registered with AHPRA as a Clinical Psychiatric Nurse
  • Experience in managing an independent caseload, and delivering treatment or care to people experiencing mental illness using a recovery-based framework
  • Experience with and understanding of interventions and services that support clients with complex interconnected needs
  • A willingness to provide support and supervision to junior staff and students

    Mercy Mental Health (South West Area Mental Health Service) is a public mental health service supporting people in south west metropolitan Melbourne with severe and complex mental illnesses by providing acute and community-based care. Services are available to adults through acute inpatient programs, residential rehabilitation programs and crisis and community recovery-focused treatment programs. We also deliver inpatient and community specialist perinatal mental health services to women and infants in western Victoria.
