    Job Title: Social Worker

    Location: Wingham, Huron and Perth Region

    Job Summary (Summary)

    The Listowel Wingham Hospitals Alliance seeks a part-time Social Worker to provide direct clinical services to patients and their families within the assessment and treatment framework of social work knowledge and skills.

    Job Description

    Responsibilities:

    • Provide consultation to physicians, nurses, and other relevant disciplines concerning the social and emotional factors affecting patient treatment.
    • Maintain close communication with the physician and other professionals directly involved in the patient's treatment.
    • Assess the psychosocial needs of patients at this hospital and aid patients in connecting with community programs to meet these needs.
    • Act as an advocate on behalf of patients and their families, both in hospital and in the community.
    • Plan and coordinate, where appropriate, programs in areas of social work expertise.
    • Maintain current and proper records necessary for effective patient treatment and collaboration.
    • Participate in staff meetings, hospital committee meetings, professional conferences, and educational workshops.
    • Lead, support, or participate in key activities or action plans as identified on applicable Operational Plans.
    • Active member of applicable team/department meetings, huddles, and patient care rounds.
    • Assist in the coordination of patient discharge by collaborating with the patient, family, and multidisciplinary team.
    • Other duties as assigned.

    Requirements:

    • Master's Degree in Social Work from an accredited School of Social Work.
    • Member of the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
    • Minimum 3 years previous related experience preferred.
    • A knowledge and appreciation of diagnostic and treatment procedures utilized by related disciplines, and an ability to work constructively with other health professionals is an asset.
    • Minimal degree of physical effort.
    • High degree of mental effort and analysis.

    Compensation and Benefits:

    Salary: $45.17 - $56.45 per hour (5-step grid based on experience)

    Terms of Employment: Part-time
